Qualificações

• Bachelor's degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience).
• 4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred.
• Demonstrated experience designing workflows for population health, care management, or value-based care programs.
• Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Responsabilidades

• Healthy Planet Build & Optimization: Design, build, and maintain Healthy Planet registries, care plans, care teams, and health maintenance topics.
• Care Coordination Workflow Design: Design and maintain care coordination workflows supporting transitions of care, discharge planning, referral management, and care team communication.
• Stakeholder Partnership & Requirements Gathering: Serve as the primary Epic liaison for Population Health, Care Management, and ACO stakeholders.
• Mentorship & Team Support: Mentor junior and mid-level analysts on Healthy Planet and care coordination build standards, testing practices, and troubleshooting techniques.
• Reporting, Governance & Release Management: Partner with reporting teams to ensure Healthy Planet and care coordination data feeds support dashboards and quality/outcomes reporting.
