CVS Health is seeking a Senior Epic Analyst to lead design, build, and optimization of Epic Healthy Planet and Care Coordination solutions. You will mentor analysts, partner with clinical leadership, and translate program strategy into scalable Epic configurations.

The role focuses on population health, care management workflows, and interoperability with Care Everywhere. A strong background in Epic, population health, and transitions of care is required.

Qualifikationen

• 4+ years of Epic experience with Healthy Planet and/or care coordination modules.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules preferred.
• Experience designing workflows for population health or value-based care.
• Understanding of transitions-of-care processes and cross-continuum care delivery.
• Bachelor’s degree or equivalent in a health/IT field.

Aufgaben

• Design, build, and optimize Healthy Planet registries, care plans, teams, and health maintenance.
• Lead workflow design for care coordination and interoperability across Epic environments.
• Partner with stakeholders to translate program goals into technical requirements.
• Mentor junior analysts and review builds prior to release.
• Support governance, release planning, and reporting data feeds (Clarity/Caboodle).
