CVS Health is seeking a Senior Epic Analyst for Healthy Planet & Care Coordination to design, build, and optimize Epic solutions that support population health management and care delivery. You will mentor junior analysts and partner with clinical leadership to translate program strategy into scalable functionality.

The role focuses on Healthy Planet registries, care plans, interoperability workflows, and transitions of care.

Qualifications

Bachelor's degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience).
4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules.
Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred.
Demonstrated experience designing workflows for population health, care management, or value-based care programs.
Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Responsibilities

Lead design, build, and optimization of Epic Healthy Planet and care coordination workflows.
Design and maintain registries, care plans, care teams, and health maintenance topics.
Partner with care management and clinical leadership to translate program strategy into scalable Epic solutions.
Mentor junior analysts and review work prior to release; contribute to SOPs and onboarding materials.
Manage stakeholder sessions, present build recommendations, and support governance and release planning.
