CVS Health Corporation seeks a Senior Epic Analyst, Healthy Planet & Care Coordination to lead the design, build, and optimization of Epic solutions for population health, care management, and cross-continuum communication.

You will mentor junior analysts, partner with care management leadership, drive testing, governance, and release planning, and ensure accurate dashboards and data feeds across the enterprise.

Qualifikationen

• Bachelor's degree in health informatics, information systems, nursing, or related field.
• 4+ years of Epic experience, primarily Healthy Planet or care coordination modules.
• Epic certification in Healthy Planet or Care Everywhere preferred.

Aufgaben

• Design, build, and maintain Healthy Planet registries, care plans, care teams, and health maintenance topics.
• Design and maintain care coordination workflows supporting transitions of care, discharge planning, referral management, and team communication.
• Serve as the primary Epic liaison for Population Health, Care Management, and ACO stakeholders, translating program goals into technical requirements.
• Mentor junior and mid-level analysts on Healthy Planet and care coordination builds, testing, and troubleshooting.
• Participate in governance, release planning, and maintain up-to-date documentation for audits.
