Job summary

CVS Health is seeking a Senior Epic Analyst to lead design, build, and optimization of Epic solutions for population health and care coordination. The role focuses on Healthy Planet and Care Everywhere workflows, collaborating with clinical leadership to translate program strategy into scalable solutions.

You will mentor junior analysts, partner with care management teams, and ensure accurate dashboards and reporting through Clarity/Caboodle and other tools.

Qualifications

• Bachelor’s degree in Health Informatics, Information Systems, Nursing, or related field.
• 4+ years of Epic experience with Healthy Planet and/or care coordination modules.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules.
• Experience designing workflows for population health, care management, or value-based care programs.
• Strong understanding of transitions‑of‑care processes and cross‑continuum care delivery.

Responsibilities

• Design, build, and optimize Healthy Planet registries, care plans, care teams, and health maintenance topics.
• Configure and optimize risk stratification tools, care gap identification, and outreach workflows within Healthy Planet.
• Lead testing and validation of Healthy Planet builds to ensure accuracy of registry logic, care plan triggers, and reporting outputs.
• Design and maintain care coordination workflows supporting transitions of care, discharge planning, referral management, and care team communication.
• Partner with care management and case management teams to streamline documentation, task management, and communication tools within Epic.
• Mentor junior and mid-level analysts on Healthy Planet and care coordination build standards, testing practices, and troubleshooting techniques.
