Qualifikationen

• Associate degree or equivalent in care coordination.
• ≥3 years care coordination in outpatient settings; 1+ year in enhanced care management.
• ≥1 year in a lead capacity.
• Experience in chronic disease management and care coordination.
• Knowledge of HIPAA Privacy and Security Rule.
• Bilingual in English and Spanish; proficient with EMR and MS Office.

Aufgaben

• Provide guidance, support, and training to the Care Management team.
• Assist with management of health center questions and conflicts.
• Monitor staff productivity and performance.
• Deliver comprehensive care management and care coordination for patients with complex needs.
