Job summary

Metropolitan Jewish Health System, Inc. is seeking a Care Management Supervisor for Elderplan/HomeFirst to oversee member care coordination across the continuum. You will work with the care team, PCP, and families to ensure high-quality care and outcomes.

The role emphasizes leadership of staff, supervision of case management activities, and attention to documentation and reporting. Previous managed care experience and NYS CSW are valued, with CCM preferred.

Qualifications

• Master of Social Work Degree or related field, or RN or CSW.
• Minimum one to three years previous management experience preferred.
• Previous managed care experience in utilization management and/or case management required.
• Current NYS CSW if CSW qualified.
• CCM preferred.

Responsibilities

• Oversee member care management with members, families, and care team coordination across the continuum.
• Coordinate with PCP, SCP, and health care team to facilitate care and supervise assessments.
• Ensure documentation quality, produce efficacy reports, and manage staff performance.

Oversees the management of member care with members, their families or significant others and coordinates with the PCP, SCP and all of the health care team facilitating the member’s care across the continuum of care. The Care Management Supervisor also acts as a resource to staff reviewing cases, and makes recommendations for change, and supervises assessments of members' needs though coordination with the care team. In addition, supervises a wholistic variety of member's needs through communication with the health care team. The CMS also ensures a high level of complication with all documentation, provides efficacy reports, and manages the performance of their direct and indirect reports.
