Job summary

Horizon Center San Diego is hiring for Enhanced Care Management to support members through comprehensive care planning and community-based services. You will coordinate with providers, perform health promotion activities, and document all interactions to ensure high-quality, coordinated care.

The role emphasizes compassion, independence, and adherence to agency policies while delivering person-centered support in a recuperative care setting in San Diego.

Qualifications

• CalAIM experience in Medi-Cal programs.
• Certificate or equivalent in a related human/social service field.
• At least one year of related community living experience.
• Strong English communication skills.
• Ability to work independently within agency policies.

Responsibilities

• Engage and support ECM program members to promote recovery and well-being.
• Monitor adherence to care plans aligned with provider recommendations.
• Provide health promotion and self-management training.
• Maintain regular contact with members, including home visits when needed.
• Coordinate care with internal staff and community providers during emergencies.
• Attend medical appointments to ensure continuity of care and document referrals.
• Ensure HIPAA compliance and maintain confidential records.
