Job summary

Pacific Health Group is seeking a Senior Lead Care Coordinator – Onboarding Specialist to set the standard of care across ECM. This field-based, hybrid role leads onboarding, mentors new staff, and ensures audit-ready documentation while navigating complex care coordination and CalAIM requirements.

You will collaborate with department leadership to refine workflows, elevate quality standards, and support culture of trauma-informed, compassionate, whole-person care across the team.

Qualifications

• Residency in the hiring county is required.
• 3–5+ years of ECM/CalAIM or complex community-based case management experience.
• Travel up to 10%–30% locally within the county.
• Background check and skills assessment required.

Responsibilities

• Lead end-to-end onboarding for new Lead Care Coordinators with CalAIM training.
• Provide field-based coaching, feedback, and mentorship.
• Drive quality and audit readiness through documentation and SOPs.
• Partner with ECM leadership to refine workflows and improve care coordination.
• Support staff during first 90 days with competency milestones and coaching.
