Job summary

Pacific Health Group is seeking an ECM Lead Care Manager Team Lead to support frontline operations and strengthen team performance in a hybrid, field-based role within the Hiring County. You will mentor LCMs, coordinate complex care, and uphold documentation standards while ensuring high-quality member care.

The position requires 3+ years in ECM or related fields, strong Medi-Cal/CalAIM knowledge, and excellent communication and leadership skills. Travel within the county is expected.

Qualifications

• 3+ years of experience in ECM, case management, social services, or healthcare coordination.
• Previous experience mentoring, training, or supporting staff strongly preferred.
• Strong understanding of Medi-Cal, CalAIM, and ECM program requirements.

Responsibilities

• Provide day-to-day leadership and guidance to Lead Care Managers (LCMs).
• Coordinate care across medical, behavioral health, and community-based services.
• Support onboarding and ongoing development of LCMs, with real-time coaching.
