Job summary

Nhcare seeks an Enhanced Care Management (ECM) Lead to help develop and implement care management operations for the ECM program and oversee daily activities. You will mentor staff, coordinate with providers and community partners, and guide the creation of comprehensive care plans for high-risk populations.

Qualifications

• Two years community outreach or behavioral health experience with high-risk populations.
• Lead experience preferred; LEAP preferred (internal).
• Valid CA driver’s license and proof of auto insurance required.
• Experience in patient-centered medical home or FQHC settings preferred.
• Experience with care coordination in ECM or similar programs preferred.

Responsibilities

• Train and mentor staff; respond to daily questions.
• Ensure ECM operations run smoothly and production goals are met.
• Develop care plans linking medical, psychiatric, social, and educational services.
• Collaborate with community partners to provide team-based care.
• Manage staffing schedules and calendars; liaise with community partners.
• Lead meetings/trainings; support quality assurance and compliance.
• Hire, transfer, reward, discipline decisions; assess training needs.
• Maintain accurate documentation for regulatory and accreditation standards.
• Conduct outreach, enrollment, risk assessments and care plans.
• Assist members in navigating the healthcare system.
