Job summary

Pacific Health Group in California is seeking an ECM Lead Care Manager Team Lead to support frontline operations, mentor Lead Care Managers, and ensure high-quality, person-centered care. The role blends field-based member support with in-office leadership to maintain workflows and guidelines.

You will oversee care coordination, ensure Medi-Cal/CalAIM compliance, and help with onboarding, coaching, and documentation across the ECM program.

Qualifications

• 3+ years of ECM or case management experience.
• Experience mentoring or training staff preferred.
• Strong knowledge of Medi-Cal, CalAIM, and ECM requirements.
• Excellent communication, leadership, organizational, and problem-solving skills.
• Ability to balance member care responsibilities with operational support duties.
• Strong documentation and technology proficiency.
• Experience working with vulnerable populations preferred.

Responsibilities

• Provide day-to-day guidance to Lead Care Managers (LCMs).
• Serve as a resource for staff questions, workflows, and ECM best practices.
• Monitor team productivity, documentation completion, and operational consistency.
• Identify and elevate concerns, risks, workflow gaps, or trends to management.
• Support coverage needs, including PTO coverage and field visits.
• Collaborate with leadership to maintain accountability and quality of care.
• Reinforce company expectations, workflows, and compliance standards.
