Job summary

Pacific Health Group is seeking an ECM Lead Care Manager (LCM) Team Lead to support frontline operations, mentor staff, and strengthen care delivery across the department. This role combines field-based member support with team development in a hybrid work setting.

You will coordinate care, supervise and coach LCMs, ensure documentation quality, and assist with onboarding and training while maintaining a reduced caseload. The position is not a people manager role.

Qualifications

• 3+ years of ECM, case management, social services, or healthcare coordination.
• Experience mentoring, training, or supporting staff preferred.
• Strong understanding of Medi-Cal, CalAIM, ECM program requirements.
• Excellent communication, leadership, organizational and problem-solving skills.
• Balance member care with operational support duties.

Responsibilities

• Provide day-to-day guidance to fellow Lead Care Managers (LCMs).
• Support onboarding and ongoing development of LCMs.
• Coordinate medical, behavioral health, and community services for members.
• Assist with transitions of care, hospital discharges, referrals, and follow-up coordination.
• Maintain compliant documentation per Medi-Cal, CalAIM, and ECM requirements.
• Support outreach and community partnership activities as needed.
