Job summary

Paylocity in California is seeking a Lead Care Manager (ECM) to provide family-centered, member-focused care coordination for children and youth enrolled in Enhanced Care Management (ECM) services.

The role involves coordinating with providers, engaging ECM-eligible members, overseeing service provision, and connecting families to social supports while adhering to managed care guidelines and HIPAA requirements.

Qualifications

• 2+ years of case management experience.

• Experience working with diverse populations.
•  
• Excellent verbal and written communication; ability to work independently.

Responsibilities

• Coordinate with members’ service providers to ensure seamless care and avoid duplication.
•     
• Actively engage ECM-eligible members.
• Oversee ECM service provision and care plan implementation.
• Connect ECM members to social services and transportation as needed.
• Advocate for ECM members with health care professionals.
• Use motivational interviewing and trauma-informed care approaches.
• Collaborate with hospital staff on discharge plans.
• Attend office visits with ECM members when required.
