Job summary

MLK Community Healthcare is seeking an RN Coordinator for the Enhanced Care Management (ECM) program in California. The role coordinates care for ECM enrollees, leads nursing assessments, and ensures follow-up with PCPs and behavioral health providers.

The RN Coordinator develops care plans, guides the interdisciplinary team, and supports transitions to less intensive community care or higher levels of care. CA RN license required.

Qualifications

• RN License at California State
• BSN preferred
• 1+ year case/care management experience preferred
• Basic Life Support (BLS) for Health Care Providers
• ACLS preferred, per hospital policy and needs

Responsibilities

• Provide clinical oversight of ECM Lead Care Managers and develop patient-centered care plans for enrollees.
• Ensure ECM members receive needed services and track progress toward goals.
• Oversee quality measures across access, clinical, and experiential benchmarks.
• Coordinate care with medical assistants, PCPs, behavioral health clinicians, and specialists.
• Engage with homeless populations and promote connections to community resources.
• Support transitions in care and medication reconciliation after hospital visits.
