Job summary

Center for Humans Services seeks a Lead Care Coordinator to manage Medi‑Cal ECM members, coordinating medical, behavioral, social, and oral health needs. You will collaborate with families, caregivers, and a multidisciplinary team to expand access and ensure DHCS ECM compliance.

The role requires a proactive, culturally responsive approach and field-based coordination across home, telehealth, and community settings.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree in social work, psychology, nursing or related field preferred.
• Minimum of two years in case management or care coordination.
• Experience with CalAIM Medi-Cal ECM/CS preferred.
• Proficiency with EHR systems and basic computer skills.

Responsibilities

• Act as primary care manager for Medi‑Cal ECM members and coordinate care.
• Develop individualized care plans addressing medical, behavioral, social needs.
• Collaborate with family, ARs, caregivers, and multidisciplinary teams.
• Ensure DHCS and MCP ECM requirements compliance and documentation.

Education

High school diploma or GED

Bachelor’s degree in social work, psychology, nursing or related field

Experience with CalAIM Medi-Cal ECM/CS

Proficiency with Electronic Health Records (EHR)
