Job summary

Fresno Mission is hiring a Lead Care Manager to coordinate Enhanced Care Management for high-need beneficiaries in Medi-Cal. The role emphasizes family-centered, relationship-based care and adherence to HIPAA/privacy while coordinating with a multidisciplinary team.

You will assess patient needs, implement care plans, and connect members to services in their community, including transportation and social supports, within managed care guidelines.

Qualifications

• AA or higher in Drug and Alcohol, Social Work, Human Services, or related field.
• Must be a mature Christian with a stated faith and able to apply it professionally.

Responsibilities

• Coordinate with member’s service providers and support network to avoid duplicate services.
• Actively engage ECM-eligible members.
• Oversee ECM services and implement care plans.
• Provide services where the member lives or seeks care within plan guidelines.
• Connect ECM members to social services and transportation.
• Advocate for ECM members with healthcare professionals.
• Utilize motivational interviewing, trauma-informed care, and harm reduction.
• Monitor adherence to treatment and medications.
• Participate in supervision meetings and maintain professional conduct.
