Job summary

Hammer & NER seeks a Lead Direct Support Professional to assist the Program Manager and provide daily support to residents with intellectual and/or developmental disabilities in Hammer & NER homes and apartments.

You will build meaningful relationships, ensure a safe living environment, and promote life skills while upholding the organization’s core values: Person-Centered, Relational, Opportunistic and Stewardship.

Qualifications

• Must be 18 years of age or older.
• Proficient in using computers and smart technology.
• Ability to lift 40lbs and assist with mobility.
• Valid driver’s license and ability to drive an organizational vehicle.
• Background check clearance by MN DHS.
• Ability to work with individuals from diverse backgrounds.
• Excellent communication and teamwork.
• Fluent reading and speaking English.

Responsibilities

• Assist the Program Manager with administrative tasks for the program.
• Provide daily support to residents in Hammer & NER homes and apartments.
• Schedule medical appointments and promote community activities.
• Train staff in documentation, procedures, and medication administration.
• Maintain a clean living environment and assist with daily living activities.
• Respond to communications via Outlook, Teams, and SharePoint.
• Ensure HIPAA confidentiality and comply with mandated reporting requirements.
• Attend meetings and support program quality of life for residents.
