Job summary

Hammer & NER is seeking a Lead Direct Support Professional to assist the Program Manager with administrative duties and provide daily support to residents with intellectual and/or developmental disabilities in Hammer & NER homes and apartments.

You will foster meaningful relationships, promote life skills, and uphold the organization’s core values: Person-Centered, Relational, Opportunistic and Stewardship.

Qualifications

• Must be 18 years of age or older.
• Proficient in using computers and/or smart technology.
• Must be cleared by a background check through the MN Department of Human Services.

Responsibilities

• Administer daily living supports and assist with activities of daily living.
• Schedule medical appointments for residents and coordinate community activities.
• Train Direct Support Professionals in documentation, emergency procedures, and use of medical equipment.
• Maintain clean living environments and ensure adherence to sanitation rules.
• Respond to communications via Outlook, Teams, and SharePoint; address complaints under Program Manager guidance.
• Monitor trainings to ensure completion by due dates and protect resident privacy (HIPAA).
