Hammer Residences, Inc. is seeking a Lead Direct Support Professional to assist the Program Manager and provide daily support to residents with intellectual and/or developmental disabilities in Hammer & NER homes and apartments.

The role emphasizes relationship-building, safety, life-skills development, and alignment with Hammer & NER’s core values: Person-Centered, Relational, Opportunistic and Stewardship. Flexible hours/days required.

Qualifications

• Must be 18 years of age or older.
• Proficient in using computers and/or smart technology.
• Must be fluent reading and speaking English.
• Must be cleared by a background check through the MN Department of Human Services.
• Requires a valid driver’s license and a driving record that meets insurance requirements; ability to drive an organizational vehicle.
• Must adhere to HIPAA guidelines and confidentiality.

Responsibilities

• Schedule medical appointments for residents.
• Coordinate community activities and events.
• Train Direct Support Professionals on procedures and medications.
• Maintain clean living environments and assist with daily living tasks.
• Respond to communications via Outlook, Teams, and SharePoint.
• Collaborate with Program Manager to address performance issues and resolve complaints.
