Job summary

TriStar Skyline Medical Center is seeking a Lead Case Manager RN to supervise and coordinate the Case Management Department. The role emphasizes staff development, education, and ensuring safe discharge plans for patients.

The Lead collaborates with nurse managers, physicians, and social services to optimize transitions of care, while aligning with policy and organizational standards.

Qualifications

• Current Tennessee RN licensure.
• 2 years of discharge planning experience.
• BSN and Certified Case Manager are valued but not required.

Responsibilities

• Provides case management services for inpatient and observation patients as assigned.
• Identifies patients at risk for adverse outcomes during transitions.
• Performs a comprehensive assessment of psychosocial, medical and discharge needs with available resources.
• Reassesses the patient’s clinical condition and develops strategies to mitigate readmission risk.
• Coordinates the plan of care and drives the discharge plan with the multidisciplinary team.
• Partners with Social Services to ensure post-acute needs are appropriate.
• Assumes timely referral to Social Services when risk factors are identified.
• Involves patient and family in identifying needs and goals.
• Evaluates progression of care using InterQual criteria and communicates issues.
• Makes appropriate referrals to third party payer and disease/case management programs.
• Facilitates patient throughput with focus on safe transition and quality.
