Qualifications

• Current Tennessee RN licensure.
• 2 years of discharge planning experience.

Responsibilities

• Provides case management services for inpatient and observation patients.
• Identifies patients at risk during transitions between levels of care.
• Performs comprehensive psychosocial, medical and discharge needs assessments.
• Reassesses clinical condition and readmission risk; develops mitigation strategies.
• Coordinates plan of care and discharge plan with the multidisciplinary team.
• Partners with Social Services to ensure appropriate post-acute care.
• Ensures timely referrals to Social Services when psychosocial risks are identified.
• Involves patient and family in identifying needs and goals.
• Documents recommendations, discharge plan, and case management activities.
• Aligns patient needs with available resources to ensure safe discharge/transition.
