TriStar Skyline Medical Center in Nashville, TN, seeks a Manager of Case Management to lead hospital-based case management, driving patient-centered care and alignment with departmental goals. You will collaborate with physicians, nurses, and the care team to optimize outcomes and resource use.

The ideal candidate holds an MSW (CSWE-accredited) with supervisory experience and an established track record in acute care.

Formación

• MSW degree with CSWE accreditation required.
• Certification in social work or case management preferred.
• 3 years of acute care social work experience preferred.
• 2 years of supervisory/leadership experience preferred.

Responsabilidades

• Serve as a role model and clinical resource for staff.
• Supervise and facilitate development of staff and oversee day-to-day department tasks.
• Ensure staff are competent in use of InterQual to progress the plan of care.
• Collaborate with physicians, nurses and care team to manage patients in a timely, appropriate manner.
• Monitor the quality of CM interactions with patients, families, physicians, and the multidisciplinary team by regularly auditing documentation.
• Prepare daily, weekly and monthly reports as assigned by the Director of Case Management.
• Seek ways to control costs without compromising patient safety, quality of care, or services delivered.
• Adhere to established policy and procedure and standards of care; escalate issues promptly through the established chain of command.
• Demonstrate knowledge of regulatory requirements, ethics and quality initiatives.
• Advocate for patients' rights, needs, and values; respect cultural and religious preferences.
