Qualifications

Responsibilities

• Conduct daily reviews of medical records to assess admission/continuation and utilization.
• Collaborate with interdisciplinary teams to coordinate patient care and resolve care transition issues.
• Develop and implement discharge plans, coordinating post-hospital placement and social services.
• Refer cases to physicians or managers when criteria are not met, ensuring timely interventions.
• Liaise with community agencies to facilitate seamless transitions for discharged patients.
• Facilitate interdisciplinary meetings to address patient needs and care planning.
