TriStar Skyline Medical Center is seeking a Lead Case Manager RN to oversee the Case Management Department. The Lead collaborates with nursing staff, physicians and Social Services to develop safe discharge plans and ensure efficient transitions of care.

Responsibilities include supervising staff, conducting performance evaluations, providing orientation, and ensuring compliance with policy while supporting patient-centered care in a busy hospital setting.

Qualifications

• Current Tennessee RN licensure.
• 2 years of discharge planning experience.
• BSN is preferred.
• Certified Case Manager is preferred.
• Previous supervisory skills are valued.

Responsabilités

• Provide case management services for inpatient and observation patients.
• Identify patients at risk for adverse outcomes during transitions of care.
• Perform psychosocial and discharge needs assessments for patients and families.
• Coordinate plan of care and drive discharge planning with multidisciplinary team.
• Refer to Social Services when psychosocial risks are identified.
• Document care coordination and discharge recommendations for the health care team.
