TriStar Skyline Medical Center in Nashville, TN, seeks a capable Lead Case Manager RN to supervise the Case Manager Program and advance discharge planning across inpatient and observation units. Qualifications

• Current Tennessee RN licensure is required.
• Minimum 2 years of discharge planning experience.
• BSN preferred; CCM and supervisory experience valued.

Responsibilities

• Provides case management services for both inpatient and observation patients as assigned.
• Identifies patients at risk for adverse outcomes during transitions.
• Performs a comprehensive psychosocial and discharge needs assessment.
• Reassesses clinical condition and develops strategies to mitigate readmission risk.
• Coordinates plan of care and facilitates discharge planning with multidisciplinary team.
• Partners with Social Services to ensure post-acute needs are addressed.
• Assumes timely referral to Social Services when risk factors are identified.
• Engages patient and family in identifying needs and goals.
• Evaluates progression of care using InterQual criteria throughout the episode of care.
• Makes referrals to third party payer and disease management programs.
• Facilitates patient throughput with focus on safe transitions.
