Job summary

Telecare Corporation is seeking a Case Manager III - Mental Health in Oakland, CA to support individuals with mental illness in their community. The role requires a relevant degree or substantial experience and involves coordinating care, transportation, and service planning.

Qualifications

• Bachelor's degree in Social Services or Human Services and 1 year of experience, or High School/GED with 4 years of direct Mental Health/community services.
• An Associate's degree may be substituted for two years of experience.
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups (weekly mileage reimbursement at IRS rate).
• Willingness to use the company vehicle to drive clients to appointments and groups.

Responsibilities

• Provide primary support and case management for individuals with mental illness and co-occurring disorders.
• Help clients live as independently as possible in their chosen community.
• Operate under direct supervision and meet position standards with additional education/experience.
