Telecare Corporation seeks a Case Manager III to support individuals with mental illness in achieving independent living in the community. Under direct supervision, you will act as a primary case manager and coordinate services to meet each person's needs.

The role requires a Bachelor's degree or equivalent experience, a valid driver's license, and willingness to use a personal vehicle to transport clients to appointments and attend meetings. Shifts are full-time with a 8:00 AM–5:00 PM schedule.

Qualifications

• Bachelor's degree in Social Services or Human Services; 1 year of experience or HS/GED with 4 years direct service in mental health/community services.
• Valid driver's license and personal vehicle insurance in your name.
• Willingness to use personal vehicle to drive clients to appointments and attend meetings.

Responsabilités

• Serve as primary support and case manager for individuals with mental illness to promote independent living.
• Under supervision, perform case management duties and coordinate services.
• Assist with transportation and linkages to housing, medical and psychiatric care as needed.
