Job summary

Qualifications

• HS/GED or higher in Social Services or related field.
• Valid driver's license and insurance in your name.
• Willingness to use personal vehicle to drive clients to appointments and groups.

Responsibilities

• Provide primary support and case management for individuals with mental illness and co-occurring disorders.
• Help clients live independently in their chosen community under supervision.
• Coordinate transportation and scheduling for appointments and activities.
