Job summary

Telecare Corporation in San Diego is hiring a Case Manager III under direct supervision to provide primary support and case management for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community. This role is distinguished by additional education, years of experience, and the ability to proficiently perform job functions.

Schedule is full-time, Monday–Friday, 8:00 AM–4:30 PM.

Qualifications

• Bachelor's degree in Social Services or Human Services with 1 year of experience, or HS/GED with 4 years of direct services in Mental Health or community services.
• Valid driver's license and personal vehicle insurance in your name.
• Willingness to use your personal vehicle to drive clients to appointments and groups (weekly mileage reimbursement at IRS rate).

Responsibilities

• Provide primary support and case management for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
• Under direct supervision, perform job functions and meet position standards.
