Job summary

Telecare Corporation in San Diego seeks a Case Manager III to work under direct supervision as a primary support and case manager for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.

Shifts Available: Full-Time; on-site. Compensation: starting wage range is $23.00 - $26.25. Telecare applies geographic differentials; pay is based on location and experience.

Qualifications

• Bachelor’s degree in Social Services or Human Services with at least 1 year of experience, or HS/GED with four years of direct mental health or community services.
• Valid and current driver’s license and personal vehicle insurance in your name.
• Willingness to use the company vehicle to transport clients.

Responsibilities

• Serve as primary support and case manager for individuals with mental illness and co-occurring disorders.
• Help clients live as independently as possible in their community.
