Job summary

Telecare Corporation is seeking a Case Manager III to serve as the primary support and case manager for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.

Under supervision, the role requires a Bachelor’s degree or equivalent and driving clients to appointments. Schedule is full-time, 8:00 AM - 5:00 PM. Starting pay ranges from $23.00 to $26.25 per hour, with geographic differentials applied.

Qualifications

• Bachelor's degree in Social Services or Human Services with 1 year of experience, or HS/GED with 4 years direct mental health/community services experience; Associate degree may substitute for 2 years experience.
