Job summary

Telecare Corporation in San Diego is seeking a Case Manager II to provide primary support and case management for individuals with mental illness, helping them live as independently as possible in their chosen community. The role is full-time, with schedule 8:00 AM to 4:30 PM, Monday through Friday.

Under supervision, you will coordinate services, drive clients when needed, and participate in a team that emphasizes compassionate behavioral health care, with a wage starting at $23.00 per hour and

Qualifications

High school diploma or GED with 2 years direct mental health/community services experience.
Valid and current driver’s license with personal vehicle insurance.
Willingness to use your personal vehicle to drive clients to appointments and attend meetings.

Responsibilities

Serve as primary support and case manager for individuals with mental illness and co-occurring disorders.
Help clients live as independently as possible in their chosen community.
Work under direct supervision to coordinate services and support.
