Job summary

Volunteers of America Los Angeles is seeking a Senior Case Manager III to act as the subject matter expert for the case management team. You will mentor staff, arrange and monitor services, and ensure plans meet client needs with appropriate follow-up.

Qualifications

• Must pass fingerprint clearance and background checks.
• Current TB screening and immunization records within 5 days of hire.
• Completed VOALA Case Management training within 6 months of hire.

Responsibilities

• Lead and mentor the Case Management team; review detailed action plans.
• Coordinate intake assessments, service planning and delivery.
• Conduct weekly case conferencing and monthly file audits.
• Collaborate with providers to link participants to resources.
• Perform on-site service delivery and field outreach as required.
• Maintain secure client records and HMIS documentation.
