Job summary

Volunteers of America Los Angeles is seeking a Senior Case Manager II to lead the case management team, ensuring service plans meet client needs and coordinating assessments, scheduling and follow-up across on-site and field settings.

Responsibilities include mentoring staff, auditing cases, linking veterans and families to resources, and ensuring documentation excellence within HMIS and program requirements.

Qualifications

• Must pass fingerprint clearance and background checks.
• TB screening and immunization records within 5 days of hire.
• Current professional license (if applicable) and BLS certification.

Responsibilities

• Lead and mentor the case management team to meet client needs.
• Coordinate assessments, service plans, scheduling, and follow-up.
• Conduct home and field visits as needed to support clients.
• Monitor service delivery quality and maintain accurate HMIS records.
• Collaborate with providers to link participants to community resources.
