Job summary

Qualifications

• Must pass fingerprint clearance and background checks.
• Minimum 2 years of case management experience.
• Ability to supervise and mentor staff.
• Experience serving veterans and homeless populations preferred.

Responsibilities

• Lead a team of Case Managers and monitor caseloads.
• Oversee intake, assessments, and service planning.
• Coordinate housing stability plans and referrals.
• Conduct outreach and field visits as needed.
