Job summary

Volunteers of America Los Angeles is seeking a Senior Case Manager II to mentor the case management team and coordinate services to meet client needs. The role blends administrative duties with field/outreach work and requires assessing client needs, developing service plans, and following up to ensure progress.

The position emphasizes on-site service delivery, outreach, and collaboration with providers to link clients to resources, with a focus on veterans and families to achieve

Qualifications

• Two years of demonstrated case management experience with high needs/high intensity of service populations.
• Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
• Master’s Degree preferred in Social Work or related field

Responsibilities

• Conducts comprehensive screenings and assessments with clients to collect functional, environmental, financial, employment, housing, educational, and health information to develop a case plan.
• Monitors case managers on a regular basis to determine quality and effectiveness of services provided.
• Assists case managers by identifying and coordinating available services necessary for veterans and families to maintain/obtain self-sufficiency.
• Develops, performs, and assists with workshops pertaining to program services and veteran households’ needs.
• Participates and assists with outreach events.
