Job summary

Volunteers of America Los Angeles is hiring a Senior Case Manager II to mentor the case management team and coordinate services for vulnerable clients in the LA area.

The role combines administrative duties with field outreach, requiring a Bachelor's degree in social sciences or related field, two years of case management experience, and relevant certifications. VOALA emphasizes comprehensive client support and community resource linkage.

Qualifications

• Bachelor’s degree in the social sciences or family therapy is required.
• Two years of demonstrated case management experience with high needs/high intensity of service populations.
• Current TB screening and Immunization Records (within 5 days of hire).
• Copy of Current Professional License (if applicable).
• Current BLS Certification (within 15 days of hire).
• Completion of all required trainings (within 30 days of hire).

Responsibilities

• Lead a team of Case Managers on their active caseload, reviewing and discussing plans of action to meet client needs.
• Arrange, coordinate, monitor, and assure all services meet client needs.
• Carry out intake, assessments, service planning and service delivery.
• Maintain HMIS data and ensure client documentation meets program requirements.
• Collaborate with other service providers to link participants to community resources.
• Conduct weekly case conferencing and monthly case file audits.
