Job summary

Volunteers of America Los Angeles is seeking a Senior Case Manager III to mentor the case management team and coordinate services to meet client needs. The role includes assessing client needs, educating on available resources, developing service plans, scheduling appointments, and ensuring follow-up to progress toward goals.

On-site duties and field/outreach work are required, with emphasis on coordinating services for veterans and families to promote self-sufficiency and stabilization.

Qualifications

• Minimum total at least five (5) years of demonstrated case management experience with high needs/high intensity of service populations.
• Bachelor’s degree from an accredited institution in the social sciences or family therapy; VOALA Case Management training within 6 months of hire.

Responsibilities

• Conducts comprehensive screenings and assessments to develop a case plan.
• Develops support systems, coordinates move-in activities, rental negotiations, and service referrals.
• May make home and field visits; monitor quality and effectiveness of services.
• Audit case files for completeness and accuracy; conduct crisis interventions as needed.
• Lead a team of Case Managers, provide intake, assessments, service planning and delivery.
• Participates in outreach events and workshops for veteran households’ needs.
