Job summary

Volunteers of America Los Angeles is seeking a Senior Case Manager to mentor the case management team and coordinate services for SSVF clients. The role involves assessing needs, educating on resources, and ensuring service plans progress on schedule.

Responsibilities include coordinating a variety of services, conducting client screenings, and maintaining secure client records, with on-site duties and outreach as needed.

Qualifications

• Bachelor's Degree in Social Work or related field; four years total education/experience.
• Completion of VOALA Case Management training within 6 months of hire.
• Minimum total 3 years of experience as a case manager.

Responsibilities

• Conduct screenings and assessments to develop a case plan.
• Coordinate and refer services for veterans and families.
• Mentor and supervise case management staff.
• Develop and update service plans and track progress.
• Schedule appointments and perform follow ups to ensure progress.
• Assist with outreach and field activities on site.
