Qualifications

• Requires Nursing degree or RN licensure with 2–4 years of related experience.
• Includes disease management and clinical judgment responsibilities.
• Experience with member outreach and care coordination is preferred.

Responsabilités

• Evaluate member needs and barriers to access appropriate care.
• Develop and coordinate care/service plans with providers and resources.
• Monitor member status and update plans as needed to meet goals.
• Provide education to members and families about care options and benefits.
