Job summary

Centene seeks a Care Manager (RN) to develop, assess, and facilitate complex care management for members with physical needs, emphasizing personalized care plans and education for members and families.

The role includes telephonic, digital, home, and site outreach; coordinating with providers to ensure timely access to services and ongoing follow-up on care plan goals, with focus on member-centered care and education on options and benefits.

Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.
• RN licensure valid in state of Georgia or compact licensure.
• 2–4 years of related experience in care management or nursing.

Responsibilities

• Evaluate member needs and recommend a care plan for outcomes.
• Develop ongoing care plans and collaborate with providers to meet needs.
• Identify barriers to care and provide appropriate interventions.
• Coordinate between member and care team for person-centered care.
• Provide ongoing follow-up and monitoring of status and goals.
• Offer resource support for local services based on assessments.
• Facilitate care management and ensure timely access to care.
• Conduct outreach via phone, digital, home, or other sites.
• Collect and maintain member information for compliance.
• Educate members and families on care options and benefits.
