• Nursing degree from accredited school or BSN required.

Responsibilities

• Develop and continuously assess ongoing care plans/service plans.
• Collaborate with providers to identify resources for members' unmet needs.
• Coordinate and manage care between member and care provider team.
• Monitor care plans and member status; revise as necessary.
• Identify problems/barriers for care management and implement interventions.
• Review member data to identify trends and improve quality care.
• Develop care plans/service plans based on referrals and assessments.
• Perform outreach to assess member needs and collaborate with resources.
• Educate members and families on healthcare options and benefits.
