Job summary

ChenMed is seeking a Primary Care Physician (PCP) to provide comprehensive care for an aging patient panel, in a value-based, outcomes-driven environment. The role entails leading a care team, ensuring high-quality, cost-effective care within ChenMed’s patient-centered model.

You will collaborate with hospitalists, specialists, and management to meet quality metrics, ensure patient safety, and promote a culture of accountability and compassionate care across ChenMed centers.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
• State Medical License required.
• Board certification preferred; Board Eligibility is required.
• Must have a current DEA number for schedule II-V controlled substances.
• Basic Life Support (BLS) certification w/in first 90 days of employment.

Responsibilities

• Provide care for patients with acute and chronic illnesses encountered in older adults.
• Take full accountability for patient care and outcomes and seek consultations when needed.
• Engage with hospitalists whenever a patient is in the hospital.
• Manage assessment, diagnosis, treatment, education, health promotion and care coordination.
