Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar required.
• Must be able to obtain a State Medical License or already have a current, active State Medical License.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar preferred; Board Eligibility required.
• Must have a current DEA number for schedule II-V controlled substances.
• BLS certification from the American Heart Association or Red Cross within 90 days.

Responsibilities

• The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
• Take full accountability for patient care and outcomes and seek specialist consultation as needed.
• Engage with the hospitalist whenever a patient is in the hospital and stay involved in the patient’s detailed care.
• Responsible for assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
• Lead their care team (MA, care facilitator, and care coordinator) for in-office patients.
• For patients unable to come to the office, engage with transitional care and other resources as available.
• Cover for other providers as needed and participate in recruiting patients and providers.
• Supervise and collaborate with APPs and remain accountable for their actions.
• Perform other duties as assigned and modified by management.
