Job summary

ChenMed is seeking a Primary Care Physician (PCP) to deliver high-quality care for older adults within a value-based model. The PCP will lead a care team, coordinate services, and ensure outcomes-driven care across a panel of patients.

Strong collaboration with hospitalists and specialists is required. Applicants must hold an MD or DO in Internal Medicine, Family Medicine, or Geriatrics with an active license and DEA number.

Qualifications

• MD/DO in Internal Medicine, Family Medicine, or Geriatrics.
• State Medical License for the state(s) of work.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• Active DEA number for schedule II–V.
• BLS certification due within 90 days.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Lead care team including care promoter and coordinator.
• Collaborate with hospitalists and other specialists as needed.
• Engage with case managers and transitional care teams.
• Recruit patients and additional providers for the center.
• Adhere to guidelines, regulatory compliance, and quality measures.
