Job summary

ChenMed LLC is seeking a Primary Care Physician (PCP) to provide high-quality care for an older adult population within a physician-led, value-based organization.

The PCP will manage a panel of up to 450 patients, lead a care team, ensure adherence to guidelines, and collaborate with center and market leadership to achieve outcomes and high-quality care. Competitive compensation and comprehensive benefits are offered.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
• Must be able to obtain a State Medical License for the state(s) in which he/she will be working.
• Board certification in Internal Medicine, Family Medicine or Geriatrics preferred; Board eligibility required.
• Must have a current DEA number for scheduled substances.
• BLS certification from AHA or Red Cross required within first 90 days of employment.

Responsibilities

• Provide care for older adult patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes; consult with specialists as needed.
• Lead the care team (MA, care facilitator, care coordinator) and coordinate care across settings.
• Engage in recruiting patients and other providers as needed; collaborate with managers and medical directors to meet guidelines.
