ChenMed, a physician-led primary care organization, seeks a Primary Care Physician to deliver high-quality care to an older adult population within our centers. The role emphasizes accountability for patient outcomes, preventive health coaching, and cost-effective care delivery in a structured team setting.

The PCP will lead a care team, collaborate with hospitalists, and engage in risk and quality management programs while upholding regulatory and company goals.

Qualifikationen

• MD or DO in Internal Medicine, Family Medicine, or Geriatrics is required.
• State medical license to practice in the applicable state is required or obtainable.
• Board certification in the relevant specialty is preferred and encouraged.

Aufgaben

• Provide care for older adult patients with acute and chronic illnesses.
• Take accountability for patient care and outcomes, seeking consultations when needed.
• Engage with hospitalists and collaborate with care teams to coordinate patient care.
• Manage assessment, diagnosis, treatment, education, and care coordination.
• Lead the office care team and coordinate care for patients unable to visit the office.
