Job summary

Alliance Health is seeking a Care Manager II to lead all communication among the care team and act as the primary contact for the member served. The role requires completing comprehensive assessments and developing unified plans of care for Tailored Plan recipients, coordinating with providers, and ensuring timely authorization of services.

Qualifications

• Master’s degree in Human Services or related field is required.
• Two years of full-time MH/SUD and/or Intellectual/Developmental Disabilities experience is required.
• Must be licensed in North Carolina (LCSW/LMFT/LCAS/LCMHC/LPA or RN with NC license).

Responsibilities

• Lead communication among care team members and serve as primary contact for the member.
• Complete comprehensive assessments and develop Plans of Care.
• Assign and coordinate care activities with Care Workers and Community Health Workers.
• Submit referrals and ensure timely authorization and service delivery.
• Provide education to members and LRPs on rights, options, and payer requirements.
• Document in JIVA and ensure state/Medicaid compliance.
• Coordinate travel for field activities as needed.
• Attend treatment meetings and advocate for appropriate level of care.
