Job summary

Qualifications

• Master's degree in Human Services or related field required.
• Must be fully or provisionally licensed in NC (LCSW/LMFT/LCAS/LCMHC/LPA) or RN with MH/SUD experience.
• Strong communication and collaboration skills required.

Responsibilities

• Conduct comprehensive assessments and develop care plans for Tailored Plan recipients.
• Coordinate with care team and providers to implement the plan.
• Document activities and ensure timely authorization and follow-up.
• Travel to offices or sites as needed for meetings.
